I’m 50/50 on “he expected it” and “his reaction time is slothlike due to drugs and brain death”. I’m leaning toward the second because a normal person would automatically react even to a staged thing.

Not defending the guy but… He’s 80, like my dad, and if he’s got Alzheimer’s like they claim he does, then like my dad who has Alzheimer’s he doesn’t respond to stuff like spilling water on himself regularly to people talking to him even to the point of yelling sometimes, so Trump not recognizing gunfire, if he’s got Alzheimer’s, makes sense… he’s not home… he’s just going through the motions and ready for dinner and waiting to be served he probably thinks that he’s getting his salad here

None of them reacted when the shots happened. It wasn’t until they saw the crowd panicking. Trump later lied in his address and claimed that him and Melania both have super-awareness and that they knew the sound could be something bad, but we have video of them not reacting to the sounds at all.
