I wonder who typed this form, almost certainly not the woman who gave birth, possibly a secretary from the doctor’s office. I feel like they were advocating for the mother in at least this small way.

It wasn’t common for people to formally name their stillborn child at that point in time, nor to have a regular funeral and burial. Those are actually more recent customs. Sensibilities have changed.
