It was 100% on purpose, in the UK 2 fingers facing forward is the same as showing the middle finger.
That’s why she stopped turning her hand for the last 2 counts.

Presenters might do a finger countdown themselves, or some other prep during the countdown. It’s not supposed to show up in the final cut ever. That’s why it’s a countdown. She’s counting down to the cut in a funny way for the crew, who has her cued. If you’re all ready to go, you can presumably make whatever faces or gestures you want (until such gestures are explicitly banned, which presumably they are after a mistake like this, however rare such technical mistakes may be). So it’s ultimately the controller/producer’s fault.
I get that the presenter is the public face, and so takes the public fall for any f-ups (and thus also has to make public apologies on behalf of the network, etc.) and their job is at the mercy of their public image. But the point is that as a technical matter, unless she somehow had the countdown purposely wrong, it’s not her fault.
