Depends on who it is. The dedicated ones use a mixture of tools which can include hand drawing, Photoshop or other image editors, etc in a workflow pipeline, iterating, editing, adjusting, and so on until they have the desired result.
Others just put in a silly prompt to get a picture they thought was neato.
