Edit: I [googled](https://share.google/TR31WdwDaG5W5WO0f) it the Pilot and Copilot are the only confirmed fatalities according to the attach source, along with 40 personnel injured among the passengers and crew.

To all who are unaware or unable to tell, this is not footage of the original incident, rather, this is some type of recreation with a simulator or maybe a game.
We learn about HRO principles in my industry that often correlate its success to the aviation industry. I didn’t see any of those HRO principles applied or close loop communication. Just a bunch of people talking in code over the speaker. What happened to clear and concise communication that was supposed to installed after the Tenerife incident.

And it is.. the trucks lights in the first part are off the whole time.
The real footage clearly shows the trucks lights on.

People are overworked, stressed and dysfunctional. Departments are understaffed . Who do we blame for the 2 lives lost?

Radio comms are not just an open voice chatroom. Only one person can get out at a time. If the truck was trying to key up to respond to the tower, it’s possible they never received the stop command.
There’s lots of ways that transmission could have gotten lost. I’m sure the NTSB investigation will be focusing on that. Nothing pushes my buttons like getting walked on, getting the busy tone, or getting quick keyed. It happens more than one may think.

I so badly want to blame the ATC operator but it’s entirely possible he was on hour 17 of an 18 hour shift alone to work the the tower which is ridiculous in terms of safety. The truck should’ve been listening closer too. If there’s one thing I’m ok with sending some extra tax dollars to, it’s overfunding ATC and other transportation safety groups. Better safe than sorry. Stop making ATC operators work til they’re falling asleep!

#1. Why are the plane and the firetruck on different radio frequencies. If they were on the same, disaster averted.
#2. There are too few air traffic controllers who work too long hours. At a super busy airport like LaGuardia an air traffic controller should work a two hour shift a day. That’s it.

Still don’t understand why traffic is allowed to cross flight pathways. I’ve seen more and more of these accidents that look preventable if only better traffic patterns were observed both in the air and on the ground.

I don’t understand how all four people on board the fire truck did not see the landing lights on an approaching plane, the illumination of a plane upon landing is one of the brightest in comparison to other sources of lighting in most airports.

This is looking like a classic “swiss cheese” situation. Combination of a controller doing the job of 2 or 3 people on his own, fire truck not responding to radio call, fire truck with no transponder so the automatic runway hold short lights didn’t activate. Just awful.
