Clearly the cardiologist,.dermatologist, podiatrist, radiology department, and ENT are all in direct competition with each other and in no way possibly working with an interrelated system.

Perhaps because they need a special kind of building, outfitted with special kinds of equipment or architecture. You can’t just put a doctor’s office anywhere. Just like you can’t just put a restaurant anywhere. Or a hair salon. All kinds of businesses need specialized buildings. It’s not rocket science. Ooh! Rocket scientists. Another profession that needs specialized buildings.
At any rate, not everything is grifting or defrauding the American people, which you can do from anywhere.
