I get that this was probably a basement bathroom or something similar, but why not have the toilet with the tank against the wall? Or perhaps there used to be a wall behind the toilet for a closet and they took it out?

If it was installed properly with the tank against the wall, that outlet for the bidet would be in the way of the tank. The bidet feature is more important than a toilet facing forwards.
Though it would be easy to move the outlet, they didn’t think so.

I think I know the reasons, because I almost had the same thing happen when renovating a toilet. Old toilet bowls have the waste pipe much further away from the wall, they don’t make them anymore. Most new toilets want the pipe horizontal, so extra space for a curve downwards is required. So, new toilets have to be 30-60 cm ahead, awkwardly. The floor is concrete, so they didn’t want to move the hole. For myself, I moved the wall 15cm inwards, then a 15cm deep box (like a wardrobe/shelf) that together fill in half the gap, and then a toilet that can take the remaining 30cm. It looks really good, even though it sacrificed a 1.5×1.5×0.3m area from the small toilet. Here they didn’t think of this solution, and pointed the toilet towards the sink perhaps for convenience. Adding a shelf behind the toilet will help.
